A comprehensive HMO plan includes unlimited fixed co-payment office visits, in-patient and out-patient hospitalization coverage, and prescriptions.

A PPO offers more flexibility and freedom of choice with rates starting as low as $31 per month depending on age.

In addition, dental insurance can be added on both PPO and HMO plans.

Once we determine the best plan and premium for you, your part would be as simple as answering the medical questions on your application.

We can accomplish this by email, fax, online application, mail, or in person.

I will stay on top of the entire underwriting process through approval, until you receive the actual policy. From there, I'll be available to answer any questions, and advise you in other areas of insurance.
